Consent to Personal Data Processing Agreement

This Consent is issued by Delta West Company Limited, operator of Nippon Health, pursuant to Decree No. 13/2023/NĐ-CP on the Protection of Personal Data. By agreeing below, you consent to Nippon Health collecting, using, storing, and transferring your personal data, including sensitive health data, for the purposes described in this document.

  1. Purpose of processing
    Your personal data will be processed to arrange and manage your Ningen Dock medical check-up and/or second-opinion service, to assess eligibility for the requested package based on the information you provide, to coordinate appointments and medical requirements with our partner hospital, to issue invoices and process payments, and to communicate updates and obtain feedback to improve our services.

  2. Categories of data
    We process identification and contact information (such as full name, date of birth, nationality, passport number, email, phone, address), health information you provide for scheduling or medical assessment (such as history, symptoms, reports, imaging, test results), payment-related information handled via our payment gateway, and technical information generated by your use of our website.

  3. Recipients and cross-border transfer
    For the purposes stated above, your data will be disclosed to NTT Medical Center Tokyo (operated by NTT East, 5-9-22 Higashi-Gotanda, Shinagawa-ku, Tokyo 141-8625, Japan) for booking and medical review. With your consent, it may also be shared with authorized interpreters or logistics partners strictly as necessary to deliver the requested service. You acknowledge and consent that your data, including health data, may be transferred from Vietnam to Japan and processed there according to the hospital’s professional confidentiality and security standards. We also engage service providers (for example, website hosting and payment processing) that may process your data under written contracts imposing confidentiality and security obligations.

  4. Retention and anonymization
    Administrative booking data is retained for up to five years after your check-up for regulatory and accounting purposes. Health-related documents are anonymized after one year unless you request ongoing coordination. When no longer necessary, data is securely deleted or irreversibly anonymized.

  5. Your rights and withdrawal of consent
    You have the right to request access to, correction of, deletion of, or restriction on the processing of your personal data, and to withdraw consent at any time by contacting contact@nippon-health.com. Withdrawal may affect our ability to complete your booking or coordinate with the hospital but will not affect processing carried out before withdrawal.

  6. Minors
    If the data subject is under 18 years old, this Consent must be provided by a parent or legal guardian. By agreeing, you confirm you are the data subject aged 18 or above, or the lawful representative of the data subject.

  7. Optional communications
    You may separately consent to receive non-essential service communications (for example, newsletters or promotions). Your decision does not affect the core booking service. Where offered, this optional consent will be collected via a distinct checkbox and can be withdrawn at any time.

  8. You may provide consent to the processing of your personal data in any of the following ways

    • Online: By submitting your information and checking “I agree to the Personal Data Protection Policy and Consent to Personal Data Processing,” you give explicit consent for us to collect and transfer your data, including to Japan.

    • Email or SMS: You may confirm by replying to our official email or message in the format we suggest.

    • Continued use or signature: If you continue using our services within seven (7) days after receiving this notice, or sign a digital consent form for sensitive health data, it is deemed full consent under Vietnamese law.
