Documentation
To process your Ningen Dock health check, we will need some documentation to be prepared and filled in before the date of check-up. This could include, but is not limited to, the following documents (* is required):
Medical referrals from your local physician
Signed consent forms on accessing your medical data for purpose of doing Ningen Dock check-up*
Signed acknowledgement note of Nippon Health’s Terms and Conditions for using the service*
Filled in medical questionnaire - arranged by the hospital; this is to make sure your check-up choices are eligible given your underlying conditions*
